The volume is an official record required under the Registration of Maternity Homes Act, 1934. It records the date of reception and discharge of the birthing mother, condition on reception, no. of previous pregnancies, name of patient's husband, occupation of husband, name and address of doctor, date and time of birth, delivery normality, sex and weight of the baby, whether premature, alive or still born, The volume also contains the name and address of the person removing the newborn from the hospital and 'the person to whose care the child is removed'. May include name and address of foster or adoptive parents where applicable. The first few folios are not properly dated, but appear to relate to 1934 or 1935.
